All fields are mandate to fill. Review the below list and attached the applicable document along with the form.

  • Valid Insurance Policies
  • Written Land-Use Permissions
  • Safety & Emergency Response Plan
  • School Operations & Training Manuals (ACI/FAI/BHPA approved)
  • Instructor Credentials, Logbooks, Medical Certificates
  • Detailed Course Syllabus
  • Documented Student Progression Method/Steps
  • Record Keeping System (student logs, maintenance logs, training reports)

Note:
1. If a single document certifies or verifies more than one of the fields referred to above, the same may be submitted to fulfill the requirement accordingly.
2. Applying school/organisation need to have active an active membership with ACI for the same sports.
3. An inspection visit by the Safety & Technical Officer may be requested for multiple sports, provided that the complete Standard Operating Procedure (SOP) has been duly followed and all requisite compliance requirements are met.
